Hook: Allow NMI merchant account to show all countries

NMI Show All Countries Request:
Add hook that will allow the following functionality to be implemented.
- In the MerchantsController we need to change the call to GetActiveCountriesbyAssociates to GetCountries so that all countries are available for selection in the Save Payment modal.
- We need a way to exclude client submitted 'blacklisted' countries from that list.
- Since not all countries currently have states associated with them we'd like the states drop down selector to dynamically change to a free form text input if a country is selected that has no states mapped to it (or we need all countries to have their states mapped)

Wondering if there is a reason for not adding all countries to "ActiveCountries" excluding the "blacklisted" countries?
We have already identified that some states are missing and we are currently in the process of adding them to all clients and we will let you know when that is complete (which should be soon).
